For the same reason the Burmese, North Koreans and Sudanese get away with it today: Governments are really, really reluctant to invade other countries, because its hard going unless it adds something to you of extreme strategic value.
There is no strategic value in any of those countries, so the west is going to sit around and pick its nose.
Note: I don't think strategic value and oil is the same thing. Israel has huge strategic value and no oil.

Because the US and China were trying to take Vietnam's shit, and Pol Pot spent a good portion of his time in power monkey wrenching the Vietnamese. So the US and China protected him during much of his reign of terror.
Kind of like the reign of terror in Gaza or Sudan today. If the government is seen to be in the strategic interests of one of the big three nations, it can usually get away with genocide.
